Metabolic bone disease preterm infants

چکیده
Small preterm infants often develop osteopenia with or without rickets and with or without fraewres. Whether these Ilone abnormalities all form part of the same disease process with a wide spectrum of presentation or whether each abnormality represents a different disease is as yet unclear. Bone mineralization depends largely on adequate supplies of calcium and phosphate. The normal intra-uterine accretion of these minerals is higher than can be achieved by feeding preterm babies postnatally with breastmilk or conventional formulas. Supplementation with calcium, phosphorus and vitamin 0 is needed to prevent the development of 'neonatal osteopathy'. The main ~ction of vitamin 0 in the preterm baby is probably to Increase the intestinal absorption of calcium and phosphorus, although it may, together with other calciotrophic hormones, have a more specific effect on bone growth.

Metabolic Bone Disease in Very Low-Birth-Weight Neonates
Background: Metabolic bone diseases (MBD), including rickets and osteopenia, are important neonatal complications among preterm infants. This study aimed to determine the prevalence and risk factors of MBD in neonates with very low birth weight (VLBW). Methods: This prospective study was conducted on VLBW infants from January 2012 to July 2013. Inclusion criteria were birth weight of ≤1500 g a...
متن کاملMetabolic Bone Disease of Prematurity: Report of Four Cases
Osteopenia of prematurity has become a common problem recently because of improved survival rates of infants with very low birth weight (VLBW). The incidence of neonatal osteopenia is inversely correlated with gestational age and birth weight. Herein, we present four cases of preterm osteopenia that were referred to the pediatric endocrinology outpatient clinic with diverse clinical and laborat...
متن کاملHigh alkaline phosphatase activity and growth in preterm neonates.
In a study on 857 infants born preterm, high peak plasma alkaline phosphatase activity was independently related to slower growth rate in the neonatal period, and to a highly significant reduction in attained length at 9 months and 18 months post term. At 18 months the deficit in body length associated with peak neonatal plasma alkaline phosphase activity of 1200 IU/l or more was 1.6 cm (95% co...
متن کاملRachitic respiratory distress in small preterm infants.
A syndrome of subacute respiratory distress, severe metabolic bone disease affecting the thoracic cage, and mild cholestasis occurred in 4 small preterm infants during the first three months of life and was associated with considerable morbidity. The early radiological features in the chest resembled thos of the Mikity-Wilson syndrome, with which the disorder may be confused. After spontaneous ...
متن کاملMetabolic bone disease of prematurity☆
Metabolic bone disease (MBD) of prematurity remains a significant problem for preterm, chronically ill neonates. The definition and recommendations for screening and treatment of MBD vary in the literature. A recent American Academy of Pediatrics Consensus Statement may help close the gap in institutional variation, but evidence based practice guidelines remain obscure due to lack of normative ...
متن کاملCopper deficiency presenting as metabolic bone disease in extremely low birth weight, short-gut infants.
Copper deficiency can cause bone lesions in infants, which might be confused with child abuse. Two extremely low birth weight preterm infants had complicated medical courses requiring prolonged parenteral nutrition for short-gut syndrome, which led to the development of cholestasis. Both had spent their entire lives in the hospital. They had been on prolonged ventilator support for chronic lung...
